Transitional Aged Youth (TAY) System of Supports (SoS)
The Transition Age Youth (TAY) System of Supports (SoS) Partnership is dedicated to creating a comprehensive integrated system of supports that assists youth, 14 to 29 years old, and their families through the transition to adulthood. Launched in May 2014, the TAY SoS Partnership supports activities that promote the development of services specific for transition age youth across Simcoe County and Muskoka District. This includes the implementation of the Transition to Independence Process (TIP) model, as well as initiatives focused on Youth Engagement and Health Equity.
The TAY SoS is a partnership that emerged out of the Systems Improvement through Service Collaborative initiative in Simcoe/Muskoka. Visit www.servicecollaboratives.ca for more information on this and other initiatives across the province.
